Bingham Family Clinic provides medical services to patients who verbally declare that they meet all of the following criteria:
Residency in Utah County for at least 90 days
Lack of Medical Insurance
Household income at or below 300% of the Federal Poverty Level
Bingham Family Clinic works in partnership with outside healthcare organizations to help provide more comprehensive healthcare services.
Each partner organization has separate eligibility requirements that may require proof of income and/or residency.
Eligibility for services depends on the level of care requested, as outlined below.
A. Clinic Visits & Basic In-Clinic Testing

B. Clinic Visits, Basic Testing & Pharmaceutical Services

Pharmaceutical Medications

Verbal declaration of lack of medical insurance

Utah County residency (90 days)
Income at or below 300% of the Federal Poverty Level
Documentation required: Yes
C. Clinic Visits, Pharmaceuticals & Outsourced Testing
Includes:
All services listed above
Outsourced diagnostic testing
Requirements:
Verbal declaration of lack of medical insurance
Proof of:
Utah County residency for at least 90 days
Income at or below 250% of the Federal Poverty Level
Documentation required: Yes
Patients are encouraged to bring proof of residency and income, when available, to support timely access to eligible healthcare services.
Accepted documents for verification need to display name, be dated within 90 days, and are as follows:

Bingham Family Clinic is committed to providing care to all eligible patients.
Patients will still be seen at the clinic despite proof of income or residency.
However, partner-provided services (pharmaceutical assistance and outsourced testing) cannot be offered without the required documentation.
Eligibility requirements and available services may change based on clinic policy or partner organization guidelines. At the earliest convenience of the clinic, all policy changes will be available for patients on the clinic website and on-site.
